note that the use of a mouth guard is strongly recommended for all players, shin pads essential. I agree
to the above details being held on computer to assist with club administration, and email address to be used
for the communication of club news and information.

I am aware that there mey be times that potographs and/or footage may be taken during matches and training sessions by approved agents and / or
Ofiicers of the club. Such images shall only be used for publicity/training purposes in accordance with the Club Policy and I give consent to such
photos/images. I hereby grant approved agents and Officers of the Club the right to use images resulting from the photo/videos. This includes any reproductions or
adaptions of the images for all general purposes ie local newspapers, local magazines, other promotional articles (inc flyers) and the Club's website.
